Our nurses provide skilled nursing care to medically fragile children from 6 weeks to 22 years of age, and young adults over age 22. Every medically fragile child will have a plan of care developed by the RN Case Manager and signed by the child’s Primary Care Physician. Examples of nursing care would include medication administration, respiratory treatments including oxygen use, trach care, enteral (gastric tube) feedings, and seizure monitoring. Our care team includes Certified Nursing Assistants (CNAs) who work alongside the nurses and teachers to assist and enhance the child’s stay at Together We Grow.

Preschool Education & Childcare

Preschool Education and Childcare

Teachers are the backbone of our educational staff. We provide a caring and nurturing environment for infants, toddlers, and preschool age children from 6 weeks to age 5. Each room is designed specifically for their age group.

Classroom teachers develop a comprehensive curriculum which enhances the children’s healthy growth and development. Our infant and preschool classrooms are inclusive of our medically fragile children. Children of all abilities grow and learn together.

Weekend Program

Weekend Program

Our centers provide overnight weekend care for enrolled medically fragile children on selected weekends throughout the year. Children may attend from Friday through Sunday. Overnight weekend care provides parents/guardians with the opportunity to temporarily rest from their role as primary caregiver.

Our weekend team of nurses and teachers provide skilled nursing care and organized activities in a safe and fun environment. Each weekend has a fun theme for scheduled activities. The children are able to see their friends when they attend Holiday celebrations, such as Halloween and Christmas, are always planned for a fun-filled weekend with their friends.

After School Program

After School Programs

Our centers provide care for school age, medically fragile children after school. Parents have the option of talking to their child’s school, arranging for transportation to our center, if provided, by school bus.

Arriving at a center after school allows time for the child to relax, stretch out, and be with their friends. When the child is picked up, much of their nursing needs are completed which can make evenings easier. After school care provides consistency for the parent, utilizes few hours and does not take away from an outside nursing shift. Teachers provide organized activities and play time after school. Medically fragile children can also attend on scheduled school breaks and summer break.

Our centers provide care for medically fragile young adults aged 22 and over, who have attended a Pediatric Day Health Center (such as Together We Grow) before the age of 22. Our adult day program provides an option for parents who are seeking care for their young adult when they are no longer public school eligible. The day program is ideal for young adults who need more skilled nursing care than can be provided in a usual adult day program. There is a curriculum plan for each center, carried out by dedicated teachers. Although self-sufficiency may not be a realistic goal for our young adults, it is what helps guide the development of the daily routine. Medically fragile young adults must have attended a Pediatric Day Health Center (such as Together We Grow) before the age of 22. The young adults also have the option of attending the Weekend Program, being included in the fun activities.

Each center is designed with a multi-sensory room which provides appropriate sensory stimulation in a controlled environment.

It is designed as a therapeutic space which can help children calm and focus. The rooms are designed and equipped to stimulate the senses of hearing, sight and touch. Sensory rooms help children develop sensory skills and ensures they have a secure environment in which to regulate themselves. The aim is to create an environment where each child feels safe and is given the opportunity to explore the space within their abilities or limitations.
