About This Program

Medically Fragile Children

Important Information about our Medically Fragile Child Care Program:

  • This program is inclusive but does not offer one-on-one care.
  • Children who have acting out or behavioral issues, or certain medical conditions, including but not limited to flight risk, severe allergies or dangerous behavior toward others, may not be appropriate for our program.
  • All children requiring skilled nursing need to be under the care of a licensed physician.
  • The primary funding source is MediCal for children under the age of 21. Your local Regional Center may also authorize hours for your child. You can discuss this with your Regional Center Service Coordinator.
  • Private insurance does not pay for care at Together We Grow.
  • For young adults over the age of 21, the primary funding source is the Regional Center in your county.

To begin the enrollment process, make an appointment for a tour. You may also Contact Us directly with any questions. You will be contacted by the RN Case Manager from the center of your choice. Please be patient as this is a lengthy process.

  1. A meeting will be set up for a tour or a complete enrollment. During the enrollment period, you will be meeting with the Case Manager and asked to provide specific information regarding your child’s medical needs. This information can include, but not be limited to, records from your child’s doctor visits, immunization records, all medications required by the child, IFSP/IEP report, MediCal card and any other insurance cards. The Case Manager will need to do a complete nursing assessment on your child.
  2. The Case Manager creates a Plan of Care and Medication Profile. This will be sent to the child’s primary physician for certification and signature.
  3. The completed enrollment packet will be submitted to MediCal for a Treatment Authorization Request to receive authorized hours to attend Together We Grow if the child is under 21 years of age. If the child is over age 21, you will have a discussion with the Case Manager regarding other funding sources such as the Regional Center.
  4. When authorization is received, your child is officially enrolled!
